What is a Transition Clinic?
Somewhere between the age of 16yr - 18yr you will transfer your care to adult services
We currently hold transition Clinics once every two months at:
1. Royal Manchester Children's Hospital
2. Royal Liverpool Children's Hospital
At this clinic you will meet the Adult Team from Manchester Royal Infirmary for the first time.
You will have your ECG and Heart Scan in the usual way and then you will see both the children's heart doctor and the adult heart doctor together. They will tell you how you are doing and what the plans are for the future.
You will receive a set of hand held notes that will include information about your heart condition etc.
You will also have the opportunity to meet the adult heart nurse who will make sure that you have understood everything and that you are very clear about how to contact them in the future.
We cannot see everyone at these clinics at the moment but we would hope in time that we will.
